Testimonial for medical indication

A testimonial for medical indication is a written statement by a general practicioner, in which an on medical data based impression concerning the patient and his or her health is given with another purpose than treatment.

The KNMG recommends treating general practicioners not to give a testimonial for medical indication concerning one's own patients. However a treating general practicioner can provide, with permission of the patient, factual medical information.
